Counseling women with multiple sclerosis regarding their risk of relapse during pregnancy and the postpartum period should they chose to forego treatment to get pregnant, have a baby and/or breastfeed is a routine part of neurology practice. This talk will review the most recent evidence of the risk of relapse during pregnancy and the postpartum period, the role of breastfeeding and easily identifiable predictors to help identify which women may be at particularly high risk of relapse. We will focus on how the evidence has shifted in contemporary MS cohorts--where MS disease modifying treatment use prior to pregnancy is common and women are being diagnosed earlier via MRIs. Case-based scenarios will be incorporated to provide practical advice from translation of the newest evidence into counseling recommendations for women with MS.
