The Buffalo Vocational Monitoring Survey (BVMS) is a tool to characterize work-challenged patients. Given the lack of an instrument in Spanish, we have proposed: to evaluate the adaptation of the Spanish version of the BVMS; to describe findings from this survey, to analyze the relationships between negative events and clinical and cognitive variables and to investigate the relationship between accommodations and clinical and cognitive variables.
61 MSpatients (93.2%RR-1.7%SP-5.1%PP) were included. 59%female; mean age: 38.89±10.38 years; education:14.18±2.57 years; EDSS:2.74±2.01; disease evolution 11.64±7.57; fatigue:4.33±3.17; depression:12.07±9.16. Outcomes measures: BVMS Survey; clinical outcomes; EDSS, BDI II & FatigueScale. Cognitive outcomes: BICAMS (SDMT-CVLT-BVMT-R); 7/24 Visuospatial Scale, PASAT2”-3” and Verbalfluency. Two factors were obtained: Clinical factor (EDSS-fatigue-depression-Cronbach´s α0.400) and Cognitive factor (SDMT-CVLT-BVMT-R-Cronbach´s α0.765). Psychometric evaluation included a professional translator, face and content validity procedure performed by an expert, a field test to verify the understanding of the contents and external criterion validity.
Regarding external criterion validity, between employees (dependent and self-employed) vs. unemployed (unemployed-retired) significant differences were obtained: Cognitive Factor (p=.022), SDMT (p=.040) , BVMT-R(p=.036), Pasat3 (p=.004), Pasat2 (p=.000) and Fatigue (p=.030), tendency in the Clinical factor (p=.051). Of 65.55% employee, 57.5% are in a dependency relationship, 32.5% are self-employed of informal works and 10% are self-employed composed of university professionals. 14.5% are unemployed of which 55.5% are due to disability. Regarding work problems, 13.5% refer decrease of scheduled work hours, 8.1% removal of job responsibilities, 5.4% asked to work additional hours and 2.7% formal discipline for a mistake. 12% of patients work additional hours per week and 22.5% miss days of work. 2 patients received harassment. Regarding job accommodations, flexible work hours, access refrigerator, memory aids and written job instructions are the most frequently used. Depression (p=.044) differentiate patients with and without negative events. Significant associations were found between accommodations and verbal memory (p=.335) and cognitive factor (p=.407).
This Spanish version is considered a new tool to monitor employment difficulties in Spanish-speaking MS patients.
