Impact of cigarette and hookah smoking on NMOSD patients´ disability status

Introduction: Few data are available to investigate the effect of smoking history on disease severity in NMOSD.
Objective: To determine the association between patients´ smoking histories and their disability status (EDSS).
Methods: This cross-sectional study was performed among 62 NMOSD patients registered in Isfahan MS Center(IMSC), Isfahan, Iran. Patients were interviewed using a structured 19-item checklist (items were designed to obtain patients´ demographic information, their EDSS, active/passive cigarette and hookah (traditional water-pipe) smoking histories). Patients´ EDSS scores were used to assess their level of disability.
Results: We enrolled 62 patients in this study of which 71% were female and 80% were married. Mean age, mean age at disease diagnosis and EDSS of patients at the time of data collection were 37.5±10.16, 32.9±10.8 and 1.8±1.6, respectively. Numbness and weakness of the limbs (43%) and optic neuritis (30%) were the two most prevalent first clinical presentations. An association between present or former passive hookah smoking and a higher EDSS among the patients was calculated (p-values< 0.05). However, no associations between having a higher EDSS and having a history of present/former active cigarette smoking, present/former passive cigarette smoking was reported (p-value>0.05). We also found no associations between present/former hookah smoking (p-value>0.05) and a higher EDSS. Analyzing the smoking duration among active and passive smokers, we also found a significantly higher EDSS for passive hookah smokers who were exposed to hookah smoking for more than 20 years (in comparison with those who were exposed for less than 10 years and 10 to 20 years).
Conclusion: Our results show that hookah smoking may aggravate NMOSD patients´ disability status(EDSS). However, cigarette smoking seems to not affect patients´ EDSS scores.
