Introduction: The prevalence of MS has sharply increased in many developing countries during the last two decades. The challenges for research and care of the patients are different in various parts of the worlds due to differences in Economics, Culture and Health system infrastructures. Here we performed a survey to investigate the major challenges in care and research in MS in Iran.

Method: To identify the most important challenges in care of the patients we used a 20 items self-reporting questionnaire. 650 patients from MS clinic of Sina hospital and MS society were included in this study. To investigate the most important problems in MS research, we used 20 items self-reporting questionnaire. The questionnaire was sent to the researchers who had at least 3 papers on MS and were cited in Pubmed. This questionnaire include 9 questions of demographic data and 19 items that ask about the priority of different research problems in Iran by Likert scale and also an open question that ask researchers to name three most research problems in MS.

Results: Regarding the most important problems in research about MS in Iran; the three most important items were “lack of awareness and compliance of patients about investigation and research”, “lack of special devoted time for research for University professors and assistants” and “long approval process for research projects”. The most important problems in MS care are the “high costs of medication”, “lack of telephone counseling (hot-line)” and “difficulty in access to proper medical team”

Conclusion: Medication costs and proper access at proper time are the most important challenges in MS care in Iran. Poor patient compliance and lack of enough devoted time for research are the most important issued raised by Iranian researchers.
